본 서류는 해당사항이 있는 분들만 작성하시면 됩니다.
예를들면, 군복무 경험이 있거나, 죄수 혹은 시민의 학대, 약탈, 또는 종교적 신성모독행위를 한 경험이 있거나, 정치적 또는 종교적 집회, 단체, 정당에 소속된 적이 있거나, 정부에서 일을 한 경험이 있는 분들의 한적용 됩니다. 해당사항이 없으시다면 작성하지 않아도 됩니다.

       형식은 0000-0000으로 표기되며 비자용지의 오른쪽 상단에 표기되어 있습니다.
       처음으로 신청하는 경우에는 UCI가 없을 수 있습니다. UCI칸에 "Not Applicable" 혹은
       "N/A"라고 적거나 온라인으로 신청접수를 하시는 경우 입력을 하시면 됩니다.
       - 온라인에서 입력이 안되는 경우에는 그냥 빈칸으로 남겨두시면 됩니다.

4. Military Service 군복무 여부
